Afterschool Ride-Home Permission & Liability Waiver
课后接送回家许可及免责协议

Permission / 接送许可： I am the parent or legal guardian of the student named above. I authorize FACES and its staff, approved volunteers, or transportation providers to arrange and provide transportation for my student from the afterschool program to the home address listed above on the requested dates.
我是上述学生的家长或法定监护人。我授权育才之家及其工作人员、经批准的志愿者或接送机构，在所申请的日期安排并提供从课后班到上述家庭地址的接送服务。

Release of Liability / 责任豁免： I understand that driving and transportation involve inherent risks, including traffic, weather, vehicle accidents, delays, and other risks of travel. To the fullest extent permitted by law, I release and agree not to hold FACES, its directors, employees, staff, volunteers, drivers, agents, and transportation providers responsible for injury, illness, death, loss, or property damage arising from or related to this transportation, except for conduct that cannot legally be released.
我了解驾车和交通运输存在固有风险，包括交通、天气、车辆事故、延误及其他旅行风险。在法律允许的最大范围内，对于因本次接送产生或与之相关的伤害、疾病、死亡、损失或财产损害，我同意免除并不追究育才之家及其董事、雇员、工作人员、志愿者、司机、代理人和接送机构的责任，但法律不允许豁免的行为除外。

Parent Responsibilities / 家长责任： I will provide accurate contact and home-address information, keep FACES informed of changes, and make sure an appropriate adult is available at the destination when required. I understand that FACES may decline or stop transportation if it determines that conditions are unsafe or the arrangement cannot be verified.
我会提供准确的联系信息和家庭地址，及时告知育才之家任何变更，并在需要时确保目的地有合适的成年人接应。我了解，如育才之家认为环境不安全或无法核实接送安排，可以拒绝或停止接送。
